Required Sports Forms
In order to be a student-athlete at Bishop England High School, you must complete the player packet including all signatures and information. The player packet contains a form for a physical to be completed after April 1 to be ready for the following sports season. Once the packet is completed, it may be dropped off in the school’s main office to the attention of Paul Runey or given directly to him in the athletic department.
Concussions are an unfortunate possibility when playing sports. Bishop England takes this risk seriously by requiring all student athletes playing a contact sport to take a baseline concussion test each year before participating in team workouts and practices. The player packet that each player and parents must sign includes the symptoms of a concussion and how concussions will be treated by our training staff. Baseline concussion tests will be administered at school throughout the year. The test takes approximately 40 minutes, and the cost for the testing is $5. Dates for administering the concussion test will be listed as they are set up throughout the year, student athletes may be informed by individual coaches.
